How website, contact and patient information is handled when you interact with LifeStyle Pharmacy Group or a selected pharmacy.

This policy explains the general privacy principles that apply to this public website, general website-administration enquiries and information handled for pharmacy services by a selected pharmacy. A selected pharmacy may provide additional information about a particular service or legal requirement when you deal with it directly.

Website administration and selected pharmacies

General website-administration enquiries and website-usage information are separate from patient pharmacy records. General website enquiries can be sent to the group contact route shown below. Each selected pharmacy is responsible for handling the patient, prescription, medicine-order, counselling and delivery records it receives or creates for its pharmacy service. Information we may handle

The information involved depends on how you use the website or pharmacy services. It may include:

  • Public-site information: browser and device information, internet protocol address, pages viewed, referring page, approximate location and cookie or analytics identifiers.
  • Contact information: your name, telephone number, email address, selected pharmacy, enquiry and related correspondence.
  • Patient and pharmacy information: identity and contact details, prescriptions, medicine history, allergies, medical-aid or payment-related information, delivery details, counselling notes and other health information you provide or that a pharmacy lawfully receives.

You may browse the public pages without actively providing contact or patient information. Information you choose to send in an email, call, message or pharmacy request is provided voluntarily. However, some information may be necessary to identify the patient, assess a prescription or medicine request, respond to an enquiry, arrange an authorised delivery or meet legal and professional duties. If necessary information is not provided, the website administrator may be unable to respond or the selected pharmacy may be unable to assess, supply or complete the requested service.

Please do not send prescriptions or detailed health information to the group's general email address unless your selected pharmacy has specifically told you to use that route.

Why information is used

Information may be used to:

  • respond to enquiries and route a request to the pharmacy you select;
  • provide, manage and document pharmacy services, including prescription assessment, medicine supply, counselling and delivery enquiries;
  • identify patients and recipients, communicate about a request and manage complaints;
  • operate, protect, measure and improve the website and its content;
  • meet professional, record-keeping, safety and legal duties; and
  • protect patients, staff, the public and the pharmacy's legitimate interests.

Pharmacy confidentiality

Prescription, medicine-order and counselling information is confidential patient information. It should be accessed only by people who need it for an authorised pharmacy purpose, with the patient's authority, or where law or a professional duty permits or requires access or disclosure.

Use the selected pharmacy's direct contact details for patient-specific or clinical questions. You may ask the pharmacy for a private counselling conversation rather than discussing health information in a public area or through the general group contact route.

Sharing information

Information may be shared only where reasonably necessary and authorised, or where required or permitted by law or professional duties. Depending on the service, recipients may include authorised staff at the selected pharmacy, a prescriber or other healthcare professional, a medical scheme, a person involved in an authorised delivery, a service provider supporting communications, website operation or analytics, and a regulator or public authority.

Service providers should receive only the information needed for the service they perform and remain subject to appropriate confidentiality and privacy obligations. Information is not disclosed for an unrelated purpose without an appropriate basis.

Cookies and analytics

The website uses Google Analytics and may use cookies or similar technologies to understand visits and how pages are used. Retention

Applicable privacy principles require personal information to be kept only for as long as it is reasonably needed for an authorised purpose or required by legal, professional, safety, accounting or record-keeping obligations. Exact periods depend on the type of record and the pharmacy service involved. When information no longer needs to be retained, the applicable requirements govern its deletion, destruction or de-identification.

Your privacy rights

Subject to applicable law, identity verification and lawful limits, you may ask whether personal information is held and request access to it, correction or deletion where applicable. You may object to certain processing and may raise a concern or lodge a complaint about how personal information has been handled.
